Appointments, Forms, and Other Requests

For New Clients
Clients can easily request appointments via the CLIENT PORTAL. The client portal will provide access for you to complete the initial intake forms, request in-person and teletherapy appointments, and send/receive encrypted messages. It takes roughly 20 minutes to complete the intake process online, and information submitted is confidential. After reviewing the forms, we will confirm the appointment time you requested, or contact you to explain why we are unable to provide services and offer alternate recommendations. We will try to respond within one business day, and you are encouraged to call if you do not receive a response by then.
If you like, you may call and request a brief consultation to discuss your case, determine if we would be a good match for you needs, and ask about current availability. However, the final decision regarding entering into a professional relationship will still require reviewing specifics of your case after you complete initial intake forms.

Please note: Clients are required to pre-pay for initial appointment (unless an alternative agreement has been made) and those who miss the initial appointment without providing 48 hours of notice will be not be eligible for a refund of fees.

For Current or Former Clients
Former clients can authorize release of their information by completing the form below and mailing to us via USPS. We also appreciate a call (858-752-3520) to notify us of the request. There may be a fee for providing records, based upon the size of your record.
