For New Clients

All initial appointments are made by phone only.  Please leave a voice message and we promise to return your call promptly.  Emails sent via non-encrypted server will receive a phone reply. When the first appointment is scheduled, you will receive a user name and temporary password to login to the CLIENT PORTAL.  The client portal will provide access for you to complete the initial intake forms, schedule or edit appointments online, and send/receive encrypted emails.

Once you have access to the client portal, please login to complete:

  • Biographical Information
  • Patient Registration

We also recommend that you read about Office Policies and Consent For Services, Notice of Privacy Practices, and Authorization for Payment.  Completing those forms and bringing them to your first appointment will streamline the process.

Clients who miss the initial appointment without providing 48 hours of notice will be not be eligible for a refund of fees.

For Current or Former Clients

The following forms can be used to request how your protected health information is treated and provides some guidance about exercising your HIPAA rights.

All records requests must be received in writing via USPS mail or by faxing completed forms to 858-452-6700.  If you mail or fax a form, please call 858-752-350 and leave message notifying us that it has been sent so that we can respond to your request as soon as possible.
